Use this checklist to assess your organisation’s compliance with NDIS Practice Standards and WHS obligations.

1. Risk Assessment & Planning

  • Home safety assessments are conducted for every participant’s home before services commence
  • Home Safety Checklists are used to identify risks in participants’ homes
  • Home Safety Risk Assessments are completed by workers to identify actions
  • Risk assessments are reviewed regularly and updated when circumstances change
  • Risks are identified and controlled for all work locations, including SIL homes, community settings, and service centres


2. Worker Safety Equipment

  • Workers have access to personal duress alarms or other safety devices
  • Safety devices are fit‑for‑purpose (GPS tracking, 24/7 monitoring, discrete activation)
  • Workers are trained in how to use safety equipment
  • Safety devices are maintained and checked regularly
  • Alternative safety measures are in place for workers in regional or remote locations (e.g., satellite‑ready devices)


3. Policies & Procedures

  • A documented risk management system is in place that effectively manages work health and safety risks
  • Worker supervision is provided and documented
  • Incident reporting pathways are clear and communicated to all workers
  • Behaviour support practices are in place and effective
  • Workplace safety policies are reviewed and updated regularly


4. Training & Competency

  • All workers are trained in health and safety procedures
  • Refresher training is provided regularly
  • De‑escalation training is provided to workers in high‑risk roles
  • Workers understand their rights to a safe workplace under the NDIS Code of Conduct
  • Workers understand their WHS responsibilities


5. Incident Reporting & Response

  • Serious incidents are reported to the NDIS Commission within required timeframes
  • Incidents are properly assessed, managed and responded to
  • Workers are supported after incidents (counselling, time off, return‑to‑work support)
  • Incident data is reviewed to identify trends and prevent recurrence
  • Lessons learned are communicated to all workers


6. Workplace Culture

  • A positive workplace culture is actively maintained
  • Workers feel able to raise concerns without fear of reprisal
  • Violence, abuse and neglect are never normalised
  • Workers are recognised for their commitment to safety
  • Feedback from workers is sought and acted upon


7. SIL & Group Home Settings (if applicable)

  • SIL homes are inspected monthly to ensure they are free from hazards
  • Evacuation and emergency procedures are tested regularly
  • New SIL Practice Standards are being implemented
  • A culture of fear is not present in SIL settings
  • Participants are supported to exercise their rights to privacy, dignity, and autonomy


8. NDIS Practice Standards Compliance

  • All NDIS Practice Standards are being met
  • Worker screening is conducted for all relevant roles
  • Quality and safe supports are provided in a safe environment
  • The NDIS Code of Conduct is understood and followed by all workers
